Once you have made your initial appointment for medical healthcare at Obelisk Urgent Care or medspa cosmetic services at Symmetry Oasis you can pre-register by downloading, printing, and completing our patient forms prior to your arrival. Doing so will help expedite the New Patient Registration process at your first visit.
